The Centers for Medicare & Medicaid Services (CMS) and Priority Health require contracted providers who see PriorityMedicare dual-eligible special needs plan (D-SNP) patients to be trained annually on their Model of Care. Priority Health’s Model of Care is a quality improvement tool that ensures the unique needs of D-SNP patients are met and describes the processes used to coordinate their care.

Training can be completed through a 15-minute on-demand webinar. Providers who register will automatically be attested, no form required. Providers can download a printable certificate upon completion of the webinar.
